How the Program Works
1. Planning Begins on Day One
Re-entry planning begins at the start of incarceration. Individuals have the opportunity to identify their needs, discuss personal goals, and begin exploring resources that may assist with their transition.
2. Preparing for Release
As release approaches, individualized plans are developed to address immediate needs and connect individuals with appropriate community services.
3. Six Months of Post-Release Case Management
Support does not end when someone walks out of the jail. The program provides six months of post-release case management to help individuals navigate challenges, follow through with referrals, connect with services, and work toward their identified goals.
